Start by filling in as much of the form as possible, but DO NOT sign it.
A copy of the front of the enrollee's current, valid driver's license, state ID, or passport must be included. A recent photo of the enrollee must also be included with the enrollment form. The photo should only include the enrollee's head and shoulders.
When finished, take the form to a notary public and sign in the notary's presence. The notary will require a valid photo ID.
Once the form is signed and notarized, mail it, the photo, and copy of the ID to:

There are four main things that an enrollee agrees to when signing the enrollment form:
An enrollee will not gamble with a state licensed entity. This includes casinos, racetracks, online sports wagering operators, and fantasy sports wagering operators.
An enrollee will not attempt to enter the gaming floor or wagering area at a state licensed casino or racetrack.
If an enrollee wagers with or enters the gaming floor of a state licensed casino or racetrack and is found, the enrollee will be removed from the facility and may face criminal trespassing charges.
An enrollee forfeits the right to any and all winnings.

Enrollees are excluded from all casino, racetrack, sports wagering, and fantasy sports operators licensed by the Iowa Racing & Gaming Commission. A self-exclusion will also apply to any operators approved in the future.
The Voluntary Self-Exclusion program doesn't prohibit an enrollee from going to hotels, restaurants, convention centers or any other non-gambling areas of a casino or racetrack. However, each facility defines and sets up its gambling areas differently. For questions about a specific area at a facility, please contact that facility directly.
An individual may enroll for a period of five years or for life. If an enrollee chooses five years, the enrollee will automatically be removed from the program after five years. If an enrollee chooses a lifetime exclusion, the enrollee cannot be removed from the program.
The only people who have access to exclusion information are the IRGC and State licensed operators. This includes casino, racetrack, sports wagering and fantasy sports wagering operators. Names and information for enrollees are not publicly available.
If a self-excluded person is found on a gaming floor or in a wagering area at a licensed casino or racetrack, they will be escorted off the property, and may be prosecuted for criminal trespassing. Any winnings will be withheld or confiscated.
All winnings withheld or confiscated from a self-excluded person go to the Iowa General Fund.

No. A self-exclusion applies to sports wagering and fantasy sports wagering, regardless of when a self-exclusion form was signed.
A self-exclusion for a 5-year term will automatically end 5 years from the date the enrollment form was signed. It isn't possible to end a 5-year term early. No exceptions can be made.
A self-exclusion signed on or after 07/01/2017 for a lifetime term cannot be ended. No exceptions can be made.
If an individual enrolled on or before 06/30/2017, the enrollee can submit a request to revoke their self-exclusion.
It can take up to 10 business days for the IRGC to process and respond to a request to revoke a self-exclusion.
It can take licensees up to 7 calendar days to update their systems with any self-exclusion changes.
If an individual is eligible to be removed from the program, a facility cannot continue the statewide self-exclusion.
However, many facilities have internal exclusions that separate from a statewide self-exclusion. If a person is self-excluded from a specific facility, they will need to contact the facility directly to discuss having it removed.
Additionally, revoking a statewide self-exclusion does not apply to involuntary bans or court orders.
No. A facility will only remove a self-exclusion from its systems based on information it receives directly form the IRGC.
When an individual enrolls in the program, a facility may exclude that individual from all of its locations, even those in other states. When someone is removed from the program, there isn’t any requirement regarding locations in other states. To discuss entering locations in other states, please contact the facility directly.
